Nurse Life Care Planning:

EKC’s Nurse Life Care Plan is a reliable tool for determining the monetary value of the lifetime medical and disability-related needs of a patient with catastrophic injuries or chronic health conditions. It is utilized by attorneys, claims administrators, healthcare economists, estate and trust managers, and others to determine the present and future value and timing of projected medical costs.

EKC’s Life Care Plan is a dynamic document based upon published standards of practice, comprehensive nursing assessment, data analysis, and research, and provides an organized, concise plan for current and future needs with associated costs for individuals who have experienced catastrophic injury or have chronic health care needs. It is developed by a Certified Nurse Life Care Planner who is uniquely trained to identify future needs across the entire spectrum of services which may be necessary to restore the patient to the highest possible level of functioning following the accident or illness.

Our Life Care Planning process features a consistent approach and methodology in effectively and accurately analyzing the needs of the patient. The steps of our process include:

• Comprehensive review of all records and supportive documentation

• Clinical interview and history with the patient and whenever possible a family member or significant contact who knew the patient both pre-and post-morbidity

• Interaction with the medical and health related treatment team to obtain answers to questions not established in the medical records review

• Research of the relevant literature to identify clinical practice guidelines to further establish needs and recommendations as well as support the medical and case management foundations of the case

• Where necessary, establish further treatment and cost data by collaborating with consulting specialists and sources

EKC’s Nurse Life Care Planners perform an objective critique of opposing or previously submitted Life Care Plans, a service that provides clients with a “second opinion” to assess the appropriateness and accuracy of an existing Life Care Plan. Such critiques include a thorough review of the Plan, including diagnoses, medical documentation, plan assumptions, projected services, frequencies, and pricing. We provide verbal or written critiques as requested.
